目的 觀察在硬膜外麻醉和全身麻醉下,腹腔鏡膽囊切除術(shù)中血漿腎上腺素(E)和去甲腎上腺素(NE)的變化。
方法 將30例擬行腹腔鏡膽囊切除(LC)患者,隨機(jī)分為全麻組(GA組,15例)和硬膜外麻醉組(EA組,15例),監(jiān)測(cè)麻醉前、氣腹前、氣腹時(shí)及術(shù)畢各時(shí)相點(diǎn)血漿NE及E濃度。
結(jié)果 EA組術(shù)中各時(shí)相點(diǎn)NE水平均無(wú)明顯變化,氣腹前及氣腹時(shí)E明顯增高(P<0.05),術(shù)畢基本恢復(fù)(P>0.05); GA組氣腹前NE及E無(wú)明顯變化,氣腹時(shí)NE和E均明顯增高(P<0.05),術(shù)畢E仍未恢復(fù)(P<0.05)。兩組比較,EA組氣腹前E明顯高于GA組,氣腹時(shí)NE明顯低于GA組(P<0.05)。
結(jié)論 兩種麻醉方法術(shù)中機(jī)體均存在明顯應(yīng)激反應(yīng),硬膜外麻醉下施行LC術(shù)中應(yīng)激較全麻為輕。
