目的:總結(jié)肝移植圍手術(shù)期應(yīng)用分子吸附循環(huán)系統(tǒng)(MARS)人工肝的經(jīng)驗(yàn)。
方法:回顧分析19例48次MARS人工肝治療的療效。
結(jié)果:單次或多次MARS人工肝治療能顯著降低血清總膽紅素、間接膽紅素、血氨、尿素氮、肌酐、尿酸等水平。19例患者中15例成功過(guò)渡到肝移植治療; 1例因免疫抑制劑中毒經(jīng)MARS人工肝治療癥狀改善; 1例移植肝功能好轉(zhuǎn); 1例晚期肝硬變患者治療后肝性腦病癥狀得到緩解,因各種原因不愿再行肝移植,出院后2周死亡; 1例患者因上消化道大出血未能等到肝移植而死亡。治療成功率為89.5%。
結(jié)論:MARS人工肝是治療肝功能衰竭安全而有效的輔助方法,為使患者能過(guò)渡到肝移植創(chuàng)造了機(jī)會(huì)。
