目的  探討多學(xué)科協(xié)作模式下經(jīng)直腸超聲(TRUS) 聯(lián)合64排多層螺旋CT(MSCT)和血清淀粉樣蛋白A(SAA)檢測的多模式術(shù)前評估系統(tǒng)對中低位直腸癌診療的臨床價值和對手術(shù)方案選擇的影響。方法  前瞻性納入2008年11月至2009年3月期間于四川大學(xué)華西醫(yī)院肛腸外科住院的中低位直腸癌患者150例(腫瘤下緣距齒狀線≤10 cm),隨機均分為MPE組(術(shù)前行TRUS、MSCT和SAA聯(lián)合評估)和MSCT+SAA組(術(shù)前行MSCT和SAA聯(lián)合評估),將術(shù)前分期和預(yù)測手術(shù)方案分別與術(shù)后病理分期和實際手術(shù)方案比較,并分析手術(shù)方案選擇與臨床病理因素的關(guān)系。結(jié)果  本研究實際納入病例146例,其中MPE組74例,MSCT+SAA組72例,2組基線情況一致。術(shù)前T、N、M和TNM分期的準(zhǔn)確度,MPE組分別為94.6%(70/74)、85.1%(63/74)、100%(74/74)和82.4%(61/74),MSCT+SAA組分別為77.8%(56/72)、84.7%(61/72)、100%(72/72)和81.9%(59/72); 2組間術(shù)前N、M和TNM分期的準(zhǔn)確度比較差異均無統(tǒng)計學(xué)意義(P gt;0.05),而2組間術(shù)前T分期的準(zhǔn)確度比較差異有統(tǒng)計學(xué)意義(P=0.003)。2組手術(shù)方案的預(yù)測符合率分別為95.9%(71/74)及88.9%(64/72),差異無統(tǒng)計學(xué)意義(P=0.106)。分析中低位直腸癌手術(shù)方案的選擇與多種臨床病理因素的關(guān)系發(fā)現(xiàn),病理學(xué)T(r=0.216,P=0.009)、N(r=0.264,P=0.001)及TNM (r=0.281,P=0.001) 分期,術(shù)前血清SAA水平(r=0.252,P=0.002)及腫瘤下緣距齒狀線距離(r=-0.261,P=0.001)與中低位直腸癌手術(shù)方案的選擇相關(guān)。結(jié)論  多模式術(shù)前評估系統(tǒng)可以實現(xiàn)準(zhǔn)確的中低位直腸癌術(shù)前分期,為手術(shù)方案的預(yù)測提供可靠的客觀依據(jù)。
