目的總結(jié)“達(dá)芬奇”機(jī)器人手術(shù)系統(tǒng)進(jìn)行普通外科手術(shù)180例的臨床經(jīng)驗(yàn)。 方法我院在2009年1月至2010年10月應(yīng)用“達(dá)芬奇”機(jī)器人手術(shù)系統(tǒng)進(jìn)行了180例普外科手術(shù),對病例的分布、手術(shù)過程以及術(shù)后恢復(fù)情況進(jìn)行總結(jié)。 結(jié)果171例患者在全機(jī)器人下完成手術(shù),9例中轉(zhuǎn)開腹手助完成,中轉(zhuǎn)率為5.0%(9/180)。行肝門部手術(shù)63例,包括肝門部膽管癌手術(shù)36例,膽囊癌手術(shù)10例,復(fù)雜肝內(nèi)膽管結(jié)石手術(shù)12例和醫(yī)源性膽管損傷手術(shù)5例; 胰腺手術(shù)44例,包括胰十二指腸切除術(shù)16例,胰體尾切除術(shù)6例,胰腺中段切除術(shù)1例,胰腺假性囊腫空腸吻合術(shù)1例和姑息性手術(shù)20例; 肝臟手術(shù)19例; 胃腸手術(shù)12例; 其他手術(shù)42例,包括膽總管探查取石術(shù)、腹膜后淋巴結(jié)清掃、脾臟切除術(shù)等。10例梗阻性化膿性膽管炎患者均在全機(jī)器人下順利完成急診手術(shù),術(shù)后感染癥狀得以控制,休克很快糾正,無圍手術(shù)期死亡病例。12例患者術(shù)后出現(xiàn)并發(fā)癥,總并發(fā)癥發(fā)生率為6.7%(12/180); 死亡2例,死亡率為1.1%(2/180)。 結(jié)論“達(dá)芬奇”機(jī)器人手術(shù)系統(tǒng)可以獨(dú)立完成各種類型普通外科手術(shù),尤其是復(fù)雜疑難的肝膽胰手術(shù),拓展了腹腔鏡外科的手術(shù)適應(yīng)證,推動(dòng)了微創(chuàng)外科的進(jìn)程。
