目的探討頻率飽和反轉(zhuǎn)恢復(fù)序列(spectral saturation inversion recovery,SPIR)、梯度回波化學(xué)位移MRI及磁共振氫譜(proton magnetic resonance spectroscopy,1H-MRS)在定量分析肝臟脂肪含量中的價值。 方法 對31例健康自愿者及22例可獲得肝臟標(biāo)本的病例行常規(guī)T1加權(quán)和T2加權(quán)(不壓脂+壓脂)、梯度回波T1加權(quán)同相位/反相位(inphase/opposedphase,IP/OP)成像及肝臟1H-MRS檢查。 測得T1WI和T2WI壓脂前、后及IP/OP的信號強度值(SInonfat1、SIfat1、SInonfat2、SIfat2、SIin及SIout),計算相對信號強度(relative signal intensity,RSI1及RSI2)和脂變指數(shù)(fat index,F(xiàn)I); 測得1H-MRS的峰值及峰下面積,計算肝細(xì)胞相對脂肪含量(relative lipid content, RLC)。 病例組22例患者于MRI掃描后接受肝臟外科手術(shù)切除,切下的肝臟進(jìn)行病理組織學(xué)檢查,并用圖像分析軟件測量肝細(xì)胞中脂變細(xì)胞百分比(proportion of fatty degenerative cells,PFDC)。 結(jié)果①脂肪肝組RSI1、FI及RLC的平均值均高于非脂肪肝組 (P lt;0.05), 但RSI2 的平均值2組間差異無統(tǒng)計學(xué)意義(P gt;0.05)。 ②脂肪肝不同病理分級間RLC的差異具有統(tǒng)計學(xué)意義,且隨脂肪肝病理級別的增加而增高(P lt;0.05); 隨著病理級別的增加,RSI1及FI逐漸增高,但差異無統(tǒng)計學(xué)意義(P gt;0.05); RSI2在脂肪肝不同病理分級間的差異亦無統(tǒng)計學(xué)意義(P gt;0.05)。 ③FI及RLC與PFDC之間存在線性正相關(guān)關(guān)系(r=0.468,P=0.027; r=0.771,P lt;0.000 1); RSI1及RSI2與PFDC之間無相關(guān)性(r=0.411,P=0.057; r=0.191,P=0.392)。 結(jié)論SPIR、梯度回波化學(xué)位移MRI及1HMRS三種方法可在一定程度上區(qū)分有無脂肪肝; 1H-MRS有助于脂肪肝的分級; 在肝臟脂肪含量定量分析方面, 1H-MRS較梯度回波化學(xué)位移MRI更具優(yōu)越性,有助與肝臟脂肪的量化分析; 而SPIR價值有限。
