目的 比較外傷性脾破裂患者與肝硬變門靜脈高壓癥患者脾切除術(shù)后血小板變化的差異,分析可能的原因及其臨床意義。
方法 統(tǒng)計(jì)我院2008年7月至2009年12月期間47例外傷性脾破裂患者及36例肝硬變患者行脾切除術(shù)前后的血小板計(jì)數(shù),比較脾切除術(shù)后2組患者血小板計(jì)數(shù)及其變化趨勢的差異。
結(jié)果 脾破裂組47例患者術(shù)后均出現(xiàn)血小板異常升高,血小板計(jì)數(shù)最高值300×109/L~600×109/L者6例,600×109/L~900×109/L
者21例,>900×109/L者20例;肝硬變組36例患者中有26例術(shù)后血小板最高值超過300×109/L,8例在100×109/L~300×109/L之間,2例仍低于100×109/L。2組患者術(shù)后血小板最高值分布的差異具有統(tǒng)計(jì)學(xué)意義(P=0.00)。脾破裂組患者與肝硬變組患者相比,術(shù)后血小板升高更明顯,下降更緩慢(P<0.05)。Child A級(jí)患者血小板異常的持續(xù)時(shí)間及血小板升高幅度均超過Child B、C級(jí)患者(P=0.006,P=0.002)。
結(jié)論 脾破裂和肝硬變患者,因肝功能及全身狀況的差異,術(shù)后血小板數(shù)量的變化有明顯的不同。術(shù)后應(yīng)根據(jù)血小板計(jì)數(shù)及凝血功能情況采取相應(yīng)的治療,方可取得良好的治療效果。
