目的 循證評價(jià)歐、美及亞洲國家和地區(qū)的專科醫(yī)師培養(yǎng)和考核標(biāo)準(zhǔn),為建立我國??漆t(yī)師制度提供決策依據(jù).
方法 應(yīng)用循證科學(xué)的原理和方法,通過PubMed、各國衛(wèi)生主管部門官方網(wǎng)站(醫(yī)師協(xié)會(huì)、??漆t(yī)師委員會(huì))、著名醫(yī)科大學(xué)和教學(xué)醫(yī)院網(wǎng)站和常用搜索引擎,系統(tǒng)檢索截止至2004年3月31日的關(guān)于專科醫(yī)師培養(yǎng)的資料,制定統(tǒng)一標(biāo)準(zhǔn),篩選和評定文獻(xiàn)質(zhì)量,并進(jìn)行綜合分析.
結(jié)果 初篩出英文文獻(xiàn)827篇,其他語種文獻(xiàn)51篇.主要涉及臨床課程(43.4%)、培訓(xùn)質(zhì)量評價(jià)(13.5%)和現(xiàn)存問題(12.7%).綜合數(shù)據(jù)庫文獻(xiàn)及官方網(wǎng)站信息發(fā)現(xiàn),各國醫(yī)師培養(yǎng)均分為院校醫(yī)學(xué)教育、職業(yè)醫(yī)學(xué)教育和繼續(xù)醫(yī)學(xué)教育3個(gè)階段,且各個(gè)階段均有嚴(yán)格標(biāo)準(zhǔn)及相應(yīng)考核.本文著重剖析了培養(yǎng)時(shí)間最長的神經(jīng)外科醫(yī)師和培養(yǎng)時(shí)間最短的家庭醫(yī)師的培養(yǎng)方案和考核標(biāo)準(zhǔn),分析??漆t(yī)師培養(yǎng)方案和標(biāo)準(zhǔn)的共同特點(diǎn).
結(jié)論 醫(yī)師培養(yǎng)是一個(gè)系統(tǒng)工程,需要國家統(tǒng)籌規(guī)劃、總體設(shè)計(jì),綜合考慮醫(yī)療市場需求、疾病負(fù)擔(dān)等因素.并建議在保證培養(yǎng)質(zhì)量的前提下,突出個(gè)性、注重發(fā)展,早日構(gòu)建符合國際規(guī)范的我國??漆t(yī)師培養(yǎng)制度.同時(shí)要轉(zhuǎn)變觀念、面對現(xiàn)實(shí),充分考慮、妥善處理好新老交替問題,邊破邊立、平穩(wěn)過渡.
