目的 通過正常活體腎臟CT/MRI圖像的測量分析,探討成人活體腎臟面積、皮質(zhì)厚度等相關(guān)指標(biāo)測定的影響因素,為影像診斷工作提供幫助.
方法 收集2004年1~4月因非腎臟疾病在我院接受腹部CT增強掃描檢查的病例106例和接受腹部MRI檢查的病例58例,分別測量雙側(cè)腎臟皮質(zhì)厚度(TC)、皮質(zhì)面積(CA)、實質(zhì)面積(PA)和腎皮質(zhì)分?jǐn)?shù)(CF),并采用SPSS 11.5統(tǒng)計軟件包進(jìn)行統(tǒng)計分析,兩組間比較用t檢驗,多組間比較用方差分析.
結(jié)果 ?、僭贑T圖像上,左腎TC、CA、PA、CF值的均數(shù)及其95%CI分別為0.62(0.44,0.80)cm、7.2(4.1,10.2)cm2、18.2(10.7,25.7)cm2、39.3(30.3,48.3)%;MRI圖像上的相應(yīng)指標(biāo)分別為0.58(0.33,0.83)cm、7.5(3.5,11.3)cm2、14.8(8.5,21.1)cm2、50.2(32.8,67.6)%.右腎在CT上的相應(yīng)指標(biāo)為0.63(0.43,0.83)cm、7.3(4,11)cm2、18.1(11,25.3)cm2、39.9(32,48)%;MRI圖像上的相應(yīng)指標(biāo)分別為0.55(0.31,0.79)cm、7.3(4.4,10.3)cm2、15.6(10.1,21.1)cm2、47.3(30,65)%.②除CF值外,不同性別及年齡組別的TC、CA、PA值的組間差異均有統(tǒng)計學(xué)意義,并且隨年齡增大而減少,女性各項指標(biāo)小于男性,CF值也有隨年齡增大而減少的趨勢.③ MRI圖像上的多數(shù)指標(biāo)小于CT的相應(yīng)指標(biāo).
結(jié)論 本組資料所顯示的TC、CA、PA及CF值等指標(biāo)能夠較好地反映成人活體腎臟的大小和功能狀態(tài),可供制定CT和MRI等影像診斷工作的正常值時參考.
