目的 探討在頦頸瘢痕粘連畸形矯正術中應用頸闊肌瘢痕組織瓣成形下頦和頸部輪廓的方法及療效。 方法2004年4月－2011年8月，收治26例頦頸及頦頸胸瘢痕粘連患者。男15例，女11例；年齡12～31歲，平均18歲。熱液燙傷19例，火焰燒傷7例。損傷分度：深Ⅱ度23例，Ⅲ度3例。傷后瘢痕形成6～27個月，平均10個月。頦頸角（117.10 ± 14.46）°，頸頦角（143.38 ± 15.68）°。頦頸瘢痕粘連畸形矯正術中，采用大小為8 cm × 7 cm～10 cm × 8 cm的頦下頸闊肌瘢痕組織瓣行頦頸部輪廓成形，中厚皮片游離移植修復頸部創(chuàng)面。 結果術后頸闊肌瘢痕組織瓣和皮片均順利成活，切口Ⅰ期愈合。患者均獲隨訪，隨訪時間6個月～3年，平均16個月。術后2周測量頦頸角為（89.31 ± 6.30）°，頸頦角為（117.83 ± 10.65）°，與術前比較差異均有統(tǒng)計學意義（P lt; 0.05），分別與正常值90°及120°比較差異均無統(tǒng)計學意義（P gt; 0.05）。21例頦頸角和頸頦角在正常范圍的患者對頦頸部外觀和頸部后仰功能恢復滿意。 結論頦下頸闊肌瘢痕組織瓣翻轉墊高下頦部，同時縮小頦頸角和頸頦角，可有效改善頦頸部外觀，是一種矯正頦頸瘢痕粘連畸形的有效方 法。
