目的 探討微型外固定架治療掌指骨開放性粉碎骨折的療效。 方法2009年10月－2011年4月，收治掌指骨開放性粉碎骨折33例。男25例，女8例；年齡21～62歲，平均25.5歲。開放性骨折Gustilo分型：Ⅱ型7例，ⅢA型26例。單純掌指骨粉碎骨折20例，合并指間或掌指關節(jié)骨折13例。肌腱部分或完全斷裂18例。傷后至手術時間2～8 h，平均3.7 h。一期清創(chuàng)復位，采用微型外固定架固定骨折。 結果術后30例患者切口Ⅰ期愈合，無釘?shù)栏腥荆?例出現(xiàn)切口感染，導致骨不連。2例掌指骨粉碎骨折發(fā)生骨不連。33例均獲隨訪，隨訪時間6個月～1年，平均8個月。X線片示13例術后8周骨折愈合，其中單純掌指骨粉碎骨折8例，合并關節(jié)骨折5例；12例術后12周愈合，其中單純掌指骨粉碎骨折7例，合并關節(jié)骨折5例；3例單純掌指骨粉碎骨折術后6個月愈合。去除外固定架后經功能鍛煉，仍有3例單純掌指骨粉碎骨折患者及4例合并關節(jié)損傷患者殘留關節(jié)僵硬，未出現(xiàn)手指旋轉及對線畸形，無慢性疼痛發(fā)生。術后6個月按照手部關節(jié)總活動度功能評分，28例骨折愈合者中，優(yōu)7例，良12例，可5例，差4例，優(yōu)良率67.9%。 結論應用微型外固定架一期治療掌指骨開放性粉碎骨折固定可靠，骨折愈合率高；波及關節(jié)的開放骨折術后功能不佳。
