目的 總結(jié)二尖瓣成形術(shù)治療二尖瓣反流患者的臨床經(jīng)驗(yàn),分析其療效?!》椒ā』仡櫺苑治?007 年1 月至 2011 年 7月中國醫(yī)科大學(xué)附屬第一醫(yī)院采用二尖瓣成形術(shù)治療89 例二尖瓣中重度反流患者的臨床資料,其中男 51例,女 38 例;年齡21~64 (48.6±13.6)歲;病程1周~21 年。術(shù)前經(jīng)食管超聲心動(dòng)圖及術(shù)中探查發(fā)現(xiàn)二尖瓣中度反流(Ⅲ級) 40 例,重度反流 (Ⅳ級) 49 例。56例二尖瓣脫垂,脫垂病變累及A2區(qū)4例、A3區(qū)5例、P2區(qū)25例、P3區(qū)14例,累及多處8例;其中二尖瓣腱索斷裂37例,腱索冗長脫垂19例;瓣環(huán)擴(kuò)大42例。手術(shù)方式:前葉腱索轉(zhuǎn)移4 例 , 前葉人工腱索 6 例 ,前葉裂縫合2例,交界部縫合 6 例 ,后葉矩形切除加 Sliding (滑行) 技術(shù)43 例,后葉折疊縫合成形14例,后葉人工腱索12例,緣對緣成形 10 例和心包補(bǔ)片修補(bǔ)瓣葉穿孔 1 例,其中2種以上修復(fù)方式9例。全部患者均使用成形環(huán)加固瓣環(huán)。手術(shù)中應(yīng)用注水試驗(yàn)和食管超聲心動(dòng)圖檢查評價(jià)成形效果?!〗Y(jié)果 無圍術(shù)期死亡。術(shù)后心臟超聲心動(dòng)圖提示:二尖瓣無反流 (0 級)15 例,微量反流 (Ⅰ級)41 例,輕度反流 (Ⅱ級) 23 例,輕至中度反流 (Ⅲ級)10 例。所有患者均無二尖瓣狹窄。術(shù)后隨訪77例(86.52%),隨訪時(shí)間3~58 (12.3±18.7) 個(gè)月。隨訪期間1例死于腦血管意外;1例行二尖瓣置換術(shù)?!〗Y(jié)論?綜合運(yùn)用多種二尖瓣成形技術(shù)治療二尖瓣中重度反流,可獲得良好的臨床療效。
