目的 采用豬右乳內(nèi)動(dòng)脈在非體外循環(huán)下行選擇性心中靜脈動(dòng)脈化(CVBG),評(píng)價(jià)其療效并對(duì)其機(jī)制進(jìn)行初步探討?!》椒ā?2只右冠狀動(dòng)脈重度彌漫性狹窄的中華實(shí)驗(yàn)小型豬 [雌雄不限,月齡7~10個(gè)月,體重(40±5) kg]按隨機(jī)數(shù)字表法分為兩組,每組6只。實(shí)驗(yàn)組:采用右乳內(nèi)動(dòng)脈行CVBG;對(duì)照組:單純開胸。右冠狀動(dòng)脈內(nèi)膜剝脫術(shù)后8周對(duì)兩組豬行超聲心動(dòng)圖檢查,術(shù)后6 h、3個(gè)月行冠狀動(dòng)脈造影、移植血管血流量測定及超聲心動(dòng)圖檢查,術(shù)后3個(gè)月處死動(dòng)物前行非放射性彩色微球檢測,觀察兩組豬心功能、移植血管血流量、移植血管通暢性和缺血心肌微循環(huán)再灌注情況?!〗Y(jié)果 右冠狀動(dòng)脈內(nèi)膜剝脫術(shù)后8周,兩組左心室功能無差異。實(shí)驗(yàn)組行CVBG術(shù)后6 h和3個(gè)月心臟收縮和舒張功能均較對(duì)照組有所改善(術(shù)后3個(gè)月射血分?jǐn)?shù) :52%±6% vs.44%±5%, t=-2.500,P=0.031)。實(shí)驗(yàn)組術(shù)后6 h、3個(gè)月移植血管血流量分別為(44.50±5.86) ml/min、(43.33±5.01) ml/min(P=0.718),搏動(dòng)指數(shù)(PI)分別為0.73±0.14、0.80±0.14 (P=0.858)。實(shí)驗(yàn)組術(shù)后6 h和3個(gè)月行冠狀動(dòng)脈造影顯示:乳內(nèi)動(dòng)脈橋及吻合口均能順利通過造影劑,無狹窄;施行CVBG后,經(jīng)非放射性彩色微球法檢測可見心肌各層的血流量均較對(duì)照組有所增加[穿壁心肌: (0.33±0.05) ml/(g ? min) vs.(0.19±0.03) ml/(g ? min),P<0.05],尤其是心內(nèi)膜下心肌。 結(jié)論 采用乳內(nèi)動(dòng)脈行CVBG,可改善心肌缺血的近、中期療效,其機(jī)制可能是通過改善心肌微循環(huán)而實(shí)現(xiàn)的。
