目的 總結(jié)非體外循環(huán)冠狀動(dòng)脈旁路移植術(shù)(OPCAB)后呼吸功能衰竭的治療經(jīng)驗(yàn)。 方法 回顧性分析2006 年6 月至2009 年12 月期間濟(jì)寧市第一人民醫(yī)院收治31 例OPCAB 后呼吸功能衰竭患者的臨床資料,其中男19 例,女12 例;年齡(62±11)歲。術(shù)前臨床診斷:穩(wěn)定型心絞痛9 例,不穩(wěn)定型心絞痛13 例,急性心肌梗死4例,陳舊性心肌梗死5 例。術(shù)前心功能分級(jí)(NYHA)Ⅰ級(jí)2 例、Ⅱ級(jí)11 例、Ⅲ級(jí)16 例,Ⅳ級(jí)2 例,合并慢性阻塞性肺病患者13 例。術(shù)后24 h 內(nèi)發(fā)生呼吸功能不全2 例,48 h 內(nèi)9 例,72 h 內(nèi)15 例,72 h 后5 例。術(shù)后48 h 因低氧血癥不能脫離呼吸機(jī)的患者主要采用高呼氣末氣道正壓(PEEP)的方法,拔除氣管內(nèi)插管6 h 后出現(xiàn)呼吸衰竭的患者給予糖皮質(zhì)激素、利尿劑、呼吸機(jī)輔助呼吸,對(duì)血流動(dòng)力學(xué)不穩(wěn)定的患者酌情給予血管活性藥物、磷酸二酯酶抑制劑、抗
心律失常藥物和使用主動(dòng)脈內(nèi)球囊反搏等。 結(jié)果 31 例患者中29 例痊愈,2 例死亡,其中1 例死于多器官功能衰竭,1 例死于急性呼吸窘迫綜合征。術(shù)后隨訪29 例,隨訪時(shí)間6 個(gè)月~ 4 年,隨訪率100%。1 例術(shù)后1 年死于腦出血,1 例術(shù)后2 年死于食管癌;3 例患者有哮喘發(fā)作,1 例患者NYHA Ⅲ~Ⅳ級(jí),2 例輕度活動(dòng)后有氣促癥狀,其余患者NYHA Ⅰ~Ⅱ級(jí),無心絞痛發(fā)作,生活質(zhì)量明顯改善。 結(jié)論 對(duì)OPCAB 后呼吸功能衰竭患者,積極改善心功能,消除肺間質(zhì)性水腫,恰當(dāng)使用呼吸機(jī)支持是治療成功的關(guān)鍵。
