病歷摘要 患者男性,65 歲。因發(fā)熱伴咳嗽、氣短8 d就診?;肌案哐獕翰 ? 年, 平素口服降壓藥, 血壓控制良好。入院體檢: 體溫38. 4 ℃, 呼吸22 次/min, 脈搏102 次/min,
血壓109 /72 mm Hg( 1 mm Hg =0. 133 kPa) , 口唇無(wú)發(fā)紺, 淺表淋巴結(jié)無(wú)腫大。頸靜脈無(wú)怒張, 左下肺叩診濁音, 余肺叩清音; 兩肺呼吸音粗, 左下肺呼吸音減弱, 可聞及少量濕啰
音。心界向左擴(kuò)大, 胸骨左緣第3 肋間可及全收縮期隆隆樣雜音3/6 級(jí), 無(wú)心包摩擦音。腹平軟, 無(wú)壓痛, 未及包塊, 肝脾肋下未及。雙下肢無(wú)腫脹。
