臨床資料  患者男性, 38 歲。主因“咳嗽, 咯痰, 活動(dòng)后氣促3 年, 加重6 個(gè)月”入院。3 年前患者于天氣變化受涼感冒后出現(xiàn)咳嗽, 咯少量痰, 自覺發(fā)熱, 當(dāng)時(shí)未測(cè)體溫, 無胸
悶、氣促、喘息、胸痛、潮熱、盜汗。3 年來患者易感冒, 活動(dòng)后氣促, 活動(dòng)耐量下降, 但無咯血、消瘦、雙下肢水腫等, 夜間喜高枕位休息, 曾在當(dāng)?shù)蒯t(yī)院診為肺結(jié)核, 行抗結(jié)核治療7個(gè)月癥狀無好轉(zhuǎn)。6 個(gè)月前患者出現(xiàn)持續(xù)性咳嗽, 咯少量痰, 活動(dòng)后氣促明顯, 無潮熱、盜汗、消瘦等, 仍診斷“肺結(jié)核”, 予口服異煙肼、利福噴丁等藥物治療3 個(gè)月, 癥狀仍無緩解, 為進(jìn)一步診治入我院。患者自15 歲后發(fā)現(xiàn)有“杵狀指”, 至本次發(fā)病期間無特殊變化。否認(rèn)其他急慢性疾病史。從事工作為廢品回收, 無放射線、重金屬毒物接觸史。吸煙4 年, 20 支/d。家族史無特殊。‥‥‥
