目的 評價呼出氣一氧化氮( eNO) 檢測對哮喘診斷的有效性和準確性 方法 采用計算機和手工檢索, 全面收集當前研究eNO檢測用于哮喘診斷的文獻, 并根據(jù)QUADAS 量表的評價標準對文獻進行評價, 用Meta-disc 軟件對可提取四格表數(shù)據(jù)的文獻進行異質(zhì)性分析, 對敏感性( Sen) 、特異性( Spe) 、診斷試驗比值比( DOR) 等結(jié)果進行合并分析, 并繪制匯總受試者工作特征曲線( SROC) 。最后進行敏感性分析。結(jié)果 共納入符合納入標準的文獻11 篇。質(zhì)量評價時, 所納入的文獻在納入人群全部經(jīng)參考標準的確認、使用相同的參考標準、待評價試驗獨立于參考標準以及所有的研究對象完成了研究這四方面較好地控制了偏倚。其次在疾病進展性偏倚發(fā)生的可能性較小,參考標準解釋可發(fā)生較大的偏倚。文獻的疾病譜、納入人群的標準、報告質(zhì)量的描述較差。在統(tǒng)計分析時, eNO檢測診斷哮喘的總的合并Sen、Spe 分別為0. 68、0. 79, DOR 為12. 73, SROC 的曲線下面積( SAUC) 為0. 8446。納入文獻穩(wěn)定性好。結(jié)論 匯總目前eNO 檢測診斷哮喘的研究顯示, eNO檢測對哮喘具有一定的診斷價值, 應(yīng)以更多高質(zhì)量的研究作進一步的分析。
