目的  通過觀察鹽酸氨溴索對吸煙大鼠氣道上皮細(xì)胞核因子κB(NF-κB)和細(xì)胞間黏附分子1(ICAM-1)表達(dá)的影響,探討氨溴索在慢性阻塞性肺疾病(COPD)治療中的抗炎作用。方法  30只Wistar大鼠隨機(jī)分為三組:①對照組:不加任何干預(yù);②吸煙組:吸煙15支/次×2次/d×6d/周×12周;③氨溴索組:吸煙情況同吸煙組,于吸煙4周后開始每日吸煙前給予鹽酸氨溴索(40 mg/kg)灌胃治療至第12周末。12周后測定氣道阻力和肺順應(yīng)性,采用免疫組織化學(xué)方法測定各組大鼠氣道上皮細(xì)胞NF-κB和ICAM-1的表達(dá)。結(jié)果  ①吸煙組和氨溴索組較對照組氣道阻力升高,肺順應(yīng)性均降低(P均 lt;0.01);其中氨溴索組較吸煙組氣道阻力降低(P lt;0.01),肺順應(yīng)性升高(P lt;0.05)。②吸煙組和氨溴索組氣道上皮細(xì)胞NF-κB和ICAM-1的表達(dá)均較對照組增高(P均 lt;0.05),其中氨溴索組較吸煙組表達(dá)降低(P均 lt;0.05)。③氣道上皮細(xì)胞NF-κB與ICAM-1的表達(dá)呈明顯正相關(guān)(r=0.924,P lt;0.01)。結(jié)論  吸煙可以使大鼠氣道阻力增高,肺順應(yīng)性降低,引起氣道上皮細(xì)胞NF-κB和ICAM-1表達(dá)升高,鹽酸氨溴索可以改善吸煙引起的肺功能減退,并可能通過影響氣道上皮細(xì)胞NF-κB和ICAM-1的表達(dá)而在COPD治療中發(fā)揮一定的抗炎作用。
