目的 研究小氣道功能與氣道敏感性和反應(yīng)性的關(guān)系,探討氣道高反應(yīng)性(BHR)變異的影響因素。方法 采用前瞻性研究,收集北京大學(xué)第三醫(yī)院2005年1月~2006年4月期間支氣管激發(fā)試驗(yàn)(BPT)陽(yáng)性疑診支氣管哮喘的患者,氣道敏感性以引起第1秒用力呼氣容積(FEV1)下降?20%的乙酰甲膽堿(Mch)累積量表示(PD20);氣道反應(yīng)性以Mch劑量反應(yīng)曲線斜率(DRS)表示,以FEV1下降率除以Mch累積量。以log10DRS作為因變量,以在50%肺活量位時(shí)的最大呼氣流速(Vmax50%)和在25%肺活量位時(shí)的最大呼氣流速(Vmax25%)反映小氣道功能,建立小氣道功能指標(biāo)與log10DRS之間的簡(jiǎn)單線性回歸模型。以log10DRS作為因變量,根據(jù)年齡、身高、FEV1%pred建立多元回歸模型,得出決定系數(shù)(r2)。將小氣道功能指標(biāo)加入此模型,重新計(jì)算r2,二者差異代表小氣道功能指標(biāo)在氣道反應(yīng)性中的作用。分別對(duì)不同性別和年齡組進(jìn)行分析。結(jié)果 共入選184例患者,男70例,女114例。PD20越低,Vmax50%和Vmax25%越低,反之亦然。PD20與log10DRS顯著負(fù)相關(guān)(r=-0.874,P lt;0.01)。簡(jiǎn)單線性回歸分析發(fā)現(xiàn),F(xiàn)EV1%、Vmax50%、Vmax25%分別與log10DNS顯著相關(guān)(決定系數(shù)r2分別為0.062,0.097和0.085,P均 lt;0.01)。與年齡、身高、FEV1%pred進(jìn)行多元線性回歸分析,Vmax50%、Vmax25%引起氣道反應(yīng)性的變異程度分別占3.9%和2.6%。不論男性還是女性患者,Vmax50%與log10DNS顯著相關(guān),并且男性r2高于女性。年齡?25歲者女性Vmax50%、Vmax25%與log10DNS顯著相關(guān),r2高于男性。而年齡在25~45歲患者男性Vmax50%、Vmax25%與log10DNS顯著相關(guān),r2高于女性。當(dāng)年齡 gt;45歲時(shí)男性和女性患者Vmax50%、Vmax25%與log10DNS均無(wú)明顯相關(guān)性。結(jié)論 小氣道功能下降導(dǎo)致氣道敏感性和反應(yīng)性增加。
