目的 探討胰十二指腸切除術(shù)胰與消化道重建方法的選擇。方法 對我院1989～1999年施行的胰十二指腸切除術(shù)后胰胃吻合83例行回顧性總結(jié)，其中行經(jīng)典的胰十二指腸切除術(shù)76例，保留幽門的胰十二指腸切除術(shù)7例。胰胃吻合是殘余胰腺與胃后壁間斷單層植入式吻合。結(jié)果 住院病死率為2.4%（2/83）； 并發(fā)癥發(fā)生率為25.3%（21/83），其中胰瘺3例，膽瘺2例，吻合口出血3例，切口裂開5例，胃排空遲緩5例，腹腔感染1例，胸腔積液1例，腸梗阻1例。結(jié)論 胰胃吻合術(shù)簡便、安全，是降低術(shù)后胰瘺的胰腸重建方法。
