目的探討胃癌患者外周血淋巴細胞(PBL)CD44變異體5(CD44v5)含量和瘤組織CD44v5表達情況,及其對胃癌轉(zhuǎn)移的診斷和進展評價的意義。方法應(yīng)用流式細胞儀(FCM)檢測31例胃癌患者術(shù)前PBL CD44v5含量并與正常對照組相比較,同時采用免疫組化SABC法檢測33例胃癌組織CD44v5表達情況。結(jié)果胃癌患者術(shù)前PBL CD44v5含量較正常對照組明顯增高(P<0.01)。有轉(zhuǎn)移的胃癌患者PBL CD44v5含量明顯高于無轉(zhuǎn)移者(P<0.01)。術(shù)前PBL CD44v5含量與腫瘤大小、浸潤深度、淋巴結(jié)轉(zhuǎn)移及病理分期呈顯著的正相關(guān)(r=0.486,P<0.05)。正常胃粘膜無CD44v5表達,胃癌組織CD44v5表達陽性率為69.7%,其表達與腫瘤浸潤深度和淋巴結(jié)轉(zhuǎn)移存在顯著相關(guān)性。胃癌原發(fā)灶CD44v5表達陽性者其淋巴結(jié)轉(zhuǎn)移陽性率顯著高于陰性者。結(jié)論CD44v5有望作為監(jiān)測胃癌轉(zhuǎn)移的生物學(xué)指標(biāo)。應(yīng)用FCM定量檢測胃癌患者術(shù)前外周血CD44v5含量,對胃癌轉(zhuǎn)移的診斷和進展的評價可能具有重要的臨床意義。
