目的 探討腹腔鏡手術(shù)中常用的氣腹介質(zhì)CO2對徑路口及腹腔內(nèi)腫瘤細胞種植及轉(zhuǎn)移的影響。
方法 手術(shù)前1 h于30只Wistar大鼠腹腔內(nèi)注入R15 肝癌細胞株,并按隨機抽樣法將大鼠均分為免氣腹組、He氣腹組和CO2氣腹組3組,實驗維持2 h、28 d后宰殺動物,比較各組徑路口及腹腔內(nèi)腫瘤的種植及轉(zhuǎn)移情況。
結(jié)果 在徑路口、腸漿膜層、腸系膜、大網(wǎng)膜和膈肌部位種植的腫瘤重量: CO2氣腹組分別為(326.7±230.3) mg、(626.2±215.9) mg、(476.2±204.8) mg、(2 536.5±906.7) mg及(384.5±149.9) mg; He氣腹組分別為(235.6±107.3) mg、(414.2±148.4) mg、(261.8±92.6) mg、(1 633.4±247.3) mg及(220.0±57.9) mg; 免氣腹組分別為(145.0±42.4) mg、(221.5±108.2) mg、(212.5±109.6) mg、(797.5±335.9) mg和113.0 mg。在各部位種植的腫瘤重量,CO2氣腹組均明顯高于He氣腹組和免氣腹組,差異有顯著性意義(P<0.05); He氣腹組均高于免氣腹組,但差異無顯著性意義(P gt;0.05)。
結(jié)論 CO2氣腹與He氣腹及免氣腹相比,可促進腹腔內(nèi)腫瘤細胞的種植與生長。
