目的 探討實時超聲造影檢測和診斷體積較小原發(fā)性肝癌的價值。
方法 分析353例共378個經(jīng)臨床和病理檢查證實的直徑≤3 cm的原發(fā)性肝癌的超聲造影表現(xiàn),包括造影的增強時相和增強方式,并與病理類型相比較。
結(jié)果 所有原發(fā)性肝癌中96.6%(365/378)的病灶動脈相顯示增強,其中 317(83.9%)個病灶增強早于肝實質(zhì),48(12.7%)個病灶增強與肝實質(zhì)同步; 增強方式中329(87.0%)個病灶呈整體增強,35(9.3%)個呈斑片狀增強,14(3.7%)個病灶呈環(huán)狀增強。在門脈相和晚期相,342(90.5%)個病灶相對于肝實質(zhì)呈低回聲。若將造影時病灶動脈相增強早于或同步于肝實質(zhì)以及門脈期消退作為超聲造影診斷肝癌的標(biāo)準(zhǔn),則敏感性為87.3% (330/378); 若將整體增強和斑片狀增強作為原發(fā)性肝癌的特異增強方式,結(jié)合增強方式和時相,則超聲造影鑒別診斷原發(fā)性肝癌的敏感性為92.9%(351/378)。
結(jié)論 超聲造影可實時動態(tài)觀察肝腫瘤相對于肝實質(zhì)的灌注和廓清過程,揭示腫瘤的血流動力學(xué),在鑒別診斷小肝癌中具有重要的臨床實用價值。
