目的研究胰頭癌患者外周血中CD4+CD25+調(diào)節(jié)性T細(xì)胞(Treg)比例及其在手術(shù)前、后的變化趨勢(shì)。方法采用流式細(xì)胞儀檢測(cè)胰頭癌患者及正常對(duì)照組外周血中CD4+CD25+調(diào)節(jié)性T細(xì)胞的數(shù)量,同時(shí)監(jiān)測(cè)CD4+/CD8+比值,并進(jìn)行手術(shù)前、后的比較。結(jié)果胰頭癌患者術(shù)前外周血中CD4+CD25+和CD4+CD25high Treg所占比例較正常對(duì)照組高(P lt;0.05),術(shù)后則出現(xiàn)不同程度下降,以術(shù)后第3天下降最明顯(P lt;0.01,P lt;0.05); 胰頭癌患者術(shù)后CA199水平低于術(shù)前,以術(shù)后第14天下降明顯(P lt;0.05)。 CD4+CD25highTreg與CA199的變化趨勢(shì)大致相同。 胰頭癌患者術(shù)前CD4+/CD8+比值比正常對(duì)照組低(P lt;0.05),手術(shù)后進(jìn)一步降低,于手術(shù)后第7天達(dá)最低(P lt;0.05)。結(jié)論胰十二指腸切除術(shù)可能有助于機(jī)體抗腫瘤免疫的恢復(fù),胰頭癌患者圍手術(shù)期可作為免疫干預(yù)的重要窗口期,CD4+CD25+調(diào)節(jié)性T細(xì)胞可作為免疫干預(yù)的靶點(diǎn)。
