目的 探討錯(cuò)配修復(fù)(MMR)基因hMLH1和hMSH2的蛋白表達(dá)與遺傳性非息肉病性結(jié)直腸癌(HNPCC)臨床病理特征及預(yù)后的關(guān)系。
方法 應(yīng)用免疫組化方法(Elivision-兩步法)檢測(cè)48例符合修訂后的 《Bethesda指南》 的HNPCC患者中hMLH1和hMSH2蛋白的表達(dá)情況,并定義兩者同時(shí)表達(dá)陽(yáng)性為MMR蛋白表達(dá),分析其表達(dá)與HNPCC臨床病理特征及預(yù)后的關(guān)系。
結(jié)果 hMLH1蛋白的表達(dá)缺失率為20.83(10/48),高于hMSH2蛋白的表達(dá)缺失率(8.33%,4/48),Ρ<0.05; MMR蛋白的表達(dá)陽(yáng)性率為70.83%(34/48)。MMR蛋白表達(dá)與腫瘤的浸潤(rùn)深度相關(guān)(Ρ<0.05)。MMR蛋白表達(dá)缺失與表達(dá)正常者的總生存率分別為85.71%(12/14)和85.29%(29/34),兩者生存曲線比較差異無(wú)統(tǒng)計(jì)學(xué)意義(Ρ>0.05)。
結(jié)論 hMLH1蛋白的表達(dá)缺失率高于hMSH2蛋白,hMLH1和hMSH2蛋白表達(dá)缺失與腫瘤的浸潤(rùn)深度相關(guān),與患者的預(yù)后無(wú)關(guān)。
