目的 探討腹股溝疝補(bǔ)片修補(bǔ)術(shù)后補(bǔ)片感染的外科處理方法和經(jīng)驗(yàn)。
方法 回顧性分析北京朝陽(yáng)醫(yī)院疝和腹壁外科2011年1月至2012年6月期間收治的67例腹股溝疝補(bǔ)片修補(bǔ)術(shù)后發(fā)生補(bǔ)片感染患者的臨床資料,對(duì)患者的傷口愈合情況加以分析。
結(jié)果 67例患者均行清除感染補(bǔ)片及周圍感染組織,未再重新放置補(bǔ)片,均行一期全層縫合傷口,同時(shí)放置傷口引流管。均成功清除了感染的補(bǔ)片;術(shù)后住院時(shí)間10~25d,平均16d;術(shù)后傷口一期愈合51例,拆線后傷口存在淺部感染、經(jīng)局部換藥后延期愈合16例。術(shù)后66例患者獲訪,隨訪時(shí)間為6~24個(gè)月(平均20個(gè)月)。隨訪期間,均無(wú)切口下積液、傷口感染、腸瘺、術(shù)后疼痛等并發(fā)癥發(fā)生,無(wú)腹股溝疝復(fù)發(fā)。
結(jié)論 腹股溝疝補(bǔ)片修補(bǔ)術(shù)后補(bǔ)片感染的外科處理十分復(fù)雜,采用去除補(bǔ)片、徹底清創(chuàng)、一期縫合及放置傷口引流管的綜合手術(shù)治療方法可以獲得比較滿意的療效。
