目的 探討人工髖關(guān)節(jié)置換術(shù)后Vancouver B1型股骨假體周圍骨折的治療方法。 方法 2006年4月-2011年2月采用記憶合金抓握式接骨板固定結(jié)合自體髂骨植骨治療6例Vancouver B1型股骨假體周圍骨折。其中男2例,女4例;年齡55~78歲,平均68.5歲。6例均為初次行人工關(guān)節(jié)置換術(shù)后6個(gè)月~3年,平均18.4個(gè)月發(fā)生假體周圍骨折;骨折至手術(shù)時(shí)間為3~6 d,平均4.2 d。 結(jié)果 術(shù)后切口均Ⅰ期愈合,無深靜脈血栓形成、肺部感染、肺栓塞等并發(fā)癥發(fā)生。6例均獲隨訪,隨訪時(shí)間13個(gè)月~4年,平均28.6個(gè)月。X線片示骨折全部愈合,愈合時(shí)間12~20周,平均14.8周。末次隨訪時(shí)Harris評(píng)分76~93分,平均83.6分;獲優(yōu)3例,良3例。無1例出現(xiàn)接骨板折斷、松動(dòng),骨折再移位、骨不連、股骨假體松動(dòng)等并發(fā)癥。 結(jié)論 記憶合金抓握式接骨板結(jié)合自體髂骨植骨治療Vancouver B1型股骨假體周圍骨折具有固定可靠、操作簡便的特點(diǎn),可獲滿意臨床療效。
